Therapy at Transformation Counseling Services

Therapy is helpful to manage life’s stressors, as well as work on increasing positive behaviors such as assertiveness. Seeking therapy is not a weakness. It is not a sign that you are defective or “crazy.” It is a clear indication—a choice—that you desire more for your life and are committed to working on becoming the best version of you possible. Seeking therapy displays strength and wisdom.

Investment for Individual Services

You have options in paying for your counseling sessions. You can incur the full cost of services—paying out of pocket—or use health insurance to pay some of the cost for sessions. If you plan to use health insurance, you may have a copay or co-insurance depending on your coverage and insurance provider. Your insurance company determines the copay fee for behavioral health services. Please verify your benefits, including deductible amount, with your insurance provider prior to your initial appointment. Also, please check with your insurance provider about teletherapy (virtual therapy) coverage.

GOOD FAITH ESTIMATE

Effective January 1, 2022, you are entitled to receive a “Good Faith Estimate” of what the charges could be for psychotherapy services provided to you. While it is not possible for a psychotherapist to know, in advance, how many psychotherapy sessions may be necessary or appropriate for a given person, this form provides an estimate of the cost of services provided. Your total cost of services will depend upon the number of psychotherapy sessions you attend, your individual circumstances, and the type and amount of services that are provided to you. This estimate is not a contract and does not obligate you to obtain any services from the provider.

This Good Faith Estimate is not intended to serve as a recommendation for treatment or a prediction that you may need to attend a specified number of psychotherapy visits. The number of visits that are appropriate in your case, and the estimated cost for those services, depends on your needs and what you agree to in consultation with your therapist. You are entitled to disagree with any recommendations made to you concerning your treatment and you may discontinue treatment at any time.

If you are paying for services privately or out of pocket, a Good Faith Estimate of the costs you may incur for psychotherapy services will be provided to you prior to your initial appointment.

INSURANCE: IN-NETWORK

I only accept the following insurances/vouchers at this time: Aetna and Loveland Foundation Vouchers.

INSURANCE: OUT-OF-NETWORK

If you have health insurance with a provider not listed, you may be able to use out-of-network benefits, in which your insurance provider may reimburse you a portion of monies spent on services. Overall, as coverage and plans vary, your provider will give you specific details about how much they reimburse, if any, for mental health or behavioral services.

PAYMENT

All fees for services, including copayments, are expected at the start of your appointment. Payments can be made using major debit and credit cards.

CANCELLATION POLICY

If you are unable to attend a session, please provide 24-hour notice to avoid a late-cancellation or no-show fee. If you do not provide advanced notice you will be charged a $75 fee. Subsequent sessions will not be scheduled until payment for all sessions is current.
